This Waiver applies to everyone registered
I am the volunteer, parent, and/or legal guardian of the volunteer (under 18) named on this form. I have the legal authority to execute this release on behalf of the volunteer. I am aware of and assume all hazards and risks associated with volunteering at this activity/event. I understand and agree that David's Table and the Host Church(es) are not responsible for the negligence or reckless conduct of others. I hereby release and hold David's Table, the Host Church(es) and all other participating volunteers harmless for any injury, claims, or damages suffered as a result of participating in this activity/event.
I authorize supervisors and any medical treatment providers to take all reasonable and necessary steps to protect the health and welfare of the volunteer in case of an accident or emergency. I authorize emergency medical treatment for the volunteer(under 18) in the event I cannot be reached for authorization.
I understand photographs and/or video of individuals may be taken during Night to Shine. I give permission to David's Table and the Host Church(es) to use the photographs and/or videos in promotional materials including, but not limited to, websites, social media, and printed material.
